Patient Information
On your first visit we will assemble
your medical and dental history. Possibly take a radiographic survey.
Full mouth x-rays are generally taken every 3-5 years and checkup
x-rays every 6-24 months, depending on your incidence of caries
(cavities). A through examination of your teeth will include an
intra oral video camera to help you see more if you like. A recording
of your gum condition, your existing restorations and cavities.
This information is used to compile a treatment plan and track your
dental and medical health. Below are a list of forms we ask our
patients to read and sign as indicated. You may download, read and
fill out before your first appointment with us; please fax ahead
or bring with you.

Comprehensive Services
to treat cavities & missing teeth:
- Fillings made of Composite (which is a tooth colored
acrylic filling material); Silver (amalgam fillings which contain
mercury) are rarely used here.
- Crowns and Bridges in Gold (~85% gold) or White
Metal (~65% gold, ~25% Platinum and Palladium) are still the most
durable restorations. Als all ceramic Crowns or Fillings which
are made of porcelain.
- Endodontic therapy - also known as a root canal
therapy to save teeth which are usually taken out due to deep
cavities or other infection problems will now last many years.
- Extractions
- Full and Partial Dentures - a removable device
to replace missing teeth; also called "plates".
- Implants - a small
cylinder of titanium metal commonly used in bone mending procedures
(ie. screws and plates used to stabilize broken bones or hip replacement
operations ) is placed through the gum tissue and allowed to integrate
with the bone, restored with a crown to replace a missing tooth

- Sedative medications and laughing gas to IV sedation
to help anxious patients relax and have a relaxed experience during
dental treatment.
Sedation Dentistry:
Why only dream
of having a beautiful smile when you can dream while getting a beautiful
smile.
Sedation Dentistry also called Conscious Sedation or Sleep Dentistry
and is a very safe and relaxing way to have your dental treatment
done; patients are very happy with the experience. First Dr. Perry
will discuss with you what is involved in doing sedation dentistry,
then carefully go through your medical history to see if it is safe
for you. You will need a companion to bring and pick-up you for
your appointment. Dr. Perry will place an IV in your arm and sedate
you, then you will sink into a very comfortable chair and covered
with a warm blanket. Your vital signs will be monitored every few
minutes. Our experienced clinical team will take care of your dental
needs. When your treatment is completed your companion will return
you home to finish your relaxing sleep.

Our Financial Policies
- We recommend that you review or call your insurance company
before your initial visit to be clear on any issues you might
have regarding coverage.
- At the time of your initial exam, our office will also review
your coverage with you, to help you understand your policy
if needed.
- Insurance payment estimates will be made for you on request.
- We do not accept all insurance policies. Please call us in
advance.
- We have financing available.
- We also conveniently accept Visa, MasterCard, American Express
and Discover cards.
